Winger looks at the broader context of 2 Corinthians 5 to show that the same passage itself provides the extent-vs-application distinction.

Verse 18: 'God reconciled us to himself.' Verse 19: 'God was reconciling the world to himself … not counting their trespasses against them.' Verse 20: 'we implore you … be reconciled to God.' The movement from v.19 (world reconciled — universal extent) to v.20 (be reconciled — individual application) embeds the extent/application contrast directly in the passage.
